Continue your Canadian PRN DE3 or EO3 subscription

ABOUT THIS CHANGE

Beginning August 3, 2026, Labtician Ophthalmics, Inc. will become the Canadian ordering and customer support provider for future PRN DE3 Omega and EO3 Omega orders.

Your existing subscription and payment information will not automatically transfer to Labtician.

To continue receiving your product through autoship, you must:

1. Review the transition and privacy information below.
2. Provide your consent.
3. Enter the information required to support the transition.
4. Continue to the secure Labtician Shopify store.
5. Sign in or create your Labtician account.
6. Select your product, quantity and available delivery frequency.
7. Enter your payment information directly through secure checkout.

Your existing EyePromise subscription will remain active during the transition. It will be cancelled only after Labtician Ophthalmics, Inc. confirms that your new autoship subscription is active. This process helps reduce the risk of missed or duplicate orders.

HOW YOUR INFORMATION WILL BE USED

The information you provide, together with relevant subscription information already held by PRN Vision Group, may be used to:

• Contact you about this Canadian subscription transition.
• Confirm your identity and subscription information.
• Help you create, activate and manage your Labtician account.
• Set up and manage your new autoship subscription.
• Process and fulfill your orders.
• Provide customer support.
• Coordinate the cancellation of your previous subscription.
• Reduce the risk of duplicate subscriptions, charges or shipments.
• Meet applicable legal, regulatory, security, accounting and recordkeeping requirements.

INFORMATION THAT MAY BE USED

Information used for the transition may include:

• Your name.
• Your email address and telephone number.
• Your shipping address and province, where required.
• Your preferred language.
• Your current PRN DE3 or EyePromise EO3 product.
• Your current quantity and delivery frequency, where available.
• Your previous or expected shipment timing, where available.
• Your registration and transition status.
• Your clinic or eye care professional, when you choose to provide it or when legally permitted and necessary.

Credit card and other payment information will not be transferred to Labtician Ophthalmics, Inc.

SERVICE PROVIDERS AND INFORMATION PROCESSING

Labtician Ophthalmics, Inc. uses approved service providers to operate its online store, process payments, manage subscriptions, communicate with customers and deliver orders.

These may include Shopify, payment processors, subscription-management providers, hosting providers, communications providers and delivery partners.

Some service providers may process information outside your province. Information processed in another jurisdiction may be subject to the laws of that jurisdiction.

Labtician Ophthalmics, Inc. remains responsible for using appropriate contractual and security safeguards for personal information under its control.
